Pricing sessions, packages and travel on one invoice
Give each session its own line with the length and date in the description — "Deep tissue massage — 60 min, 14 Mar" — so the client can read the invoice back against their calendar. Different lengths are different rates, so a 90-minute session belongs on its own line rather than as 1.5 of a 60-minute one. A package is just quantity times rate: five sessions at $95 goes in as quantity 5 at 95, and if you take something off for buying five at once, apply it once with the discount field so the PDF shows both the standing rate and the saving. Add-ons — hot stone, cupping, an extra 15 minutes — sit on their own lines at their own prices, and a house call or on-site visit gets a separate travel line so the client sees table time priced apart from the drive.
This produces a plain client invoice, not an insurance superbill. There are no fields for CPT or procedure codes, a diagnosis, a referring provider or an NPI, so if a client intends to claim the cost back from an insurer or an auto-injury adjuster, ask them what their plan actually requires before you send anything — most want a superbill generated by clinical software. What you can do here is put your license number, the dates of service and your business tax ID in the notes field, which is usually what an HSA or FSA administrator asks to see; the plan still decides what it accepts, and none of it turns this into a claim form.
The notes field also carries the terms that stop arguments later: your cancellation window, what a no-show is charged, how long a package stays valid, and whether a gift certificate was redeemed against this invoice. The currency is set to USD with no tax rate filled in, because massage is taxed as a personal service in some states and exempt in others — sometimes depending on whether the work was prescribed. If you do collect it, add one tax line with your own label and rate, and it is charged on the subtotal after any discount.
Pricing a package, an add-on and a house call
A therapist in Asheville bills a regular client at the end of March. The five-session package is entered as five units at the standing $95 rate rather than as one lump sum, so the per-session price stays visible and any loyalty reduction can come off the subtotal instead of hiding in the rate. The 90-minute booking sits on its own line because longer table time is its own rate, not one and a half of a shorter one, and the hot stone add-on is priced separately again. The last line is the drive out to the client's home — charged once, whatever happened on the table.
| Description | Qty | Rate | Amount |
|---|---|---|---|
| Deep tissue massage — 60 min, five-session package | 5 | $95.00 | $475.00 |
| Swedish massage — 90 min, 22 Mar | 1 | $135.00 | $135.00 |
| Hot stone add-on — 22 Mar session | 1 | $20.00 | $20.00 |
| Travel to client's home — Weaverville, 22 Mar | 1 | $35.00 | $35.00 |
| Subtotal | $665.00 | ||
| Total due | $665.00 | ||
Keeping table time, add-ons and travel on separate lines means a client checking the invoice can see exactly what the drive cost and what the hands-on time cost.
Five things your massage invoice should settle
Session length in the description
Sixty and ninety minutes are different rates, so give each length its own line rather than typing 1.5 in the quantity column — the client should be able to match every line to an appointment.
What the package covers and when it expires
Enter a package as quantity times your per-session rate, then state the expiry window in the notes; unused sessions surfacing six months later are the argument you are trying to avoid.
Cancellations and no-shows
Write the notice period and the charge in the notes on every invoice, and bill a missed appointment as its own dated line so it reads as your stated policy rather than a mistake.
Travel priced separately
House calls, hotel visits and corporate chair sessions each carry a trip cost — put it on its own line with the location in the description instead of quietly raising the session rate.
License number in the notes
There is no field for your state license or business tax ID, so add both to the notes; clients claiming through an HSA or FSA are usually asked for them, even though this is not a superbill.

Frequently asked questions
Can I use this as a superbill for insurance?
No — a superbill needs CPT codes, a diagnosis code, your NPI and the referring provider, and this tool has no fields for any of them. Use your clinical software for insurance work and this for straightforward client billing; if the client only needs proof of payment for an HSA or FSA, put your license number and the dates of service in the notes.
How do I bill a five-session package?
One line, quantity 5, at your per-session rate — the multiplication is done for you and the client can still see what a single session costs. Put any reduction for buying ahead in the discount field, and state the expiry window in the notes.
Do I have to charge sales tax on massage?
It depends on the state — some tax massage as a personal service, some exempt it entirely, and some exempt it only when it is medically prescribed. Leave the tax rate empty if you are not required to collect it, and check with an accountant in your state.
How do I charge for a house call or corporate chair massage?
Give travel its own line — "Travel to client's home — Weaverville" at a quantity of 1 — and keep the session at your normal rate. For on-site chair massage, put the hours in the quantity column and name the site and date in the description.
