Billing visits when the patient is paying you directly
Chiropractic billing is visit-based, so the quantity column does most of the work: eight adjustments over six weeks is one line at quantity 8 against your per-visit rate, with the date range in the description, not eight near-identical lines. Keep everything that is not an adjustment on its own line — the initial examination, the X-ray series, a re-examination halfway through the plan, and any supports or supplies handed over at reception. If the patient prepaid a care plan, invoice it as a single line with the number of visits it covers named in the description, and put the reduction for paying up front in the discount field so the PDF shows the per-visit value as well as the saving.
This is a patient statement, not an insurance claim. There are no fields for CPT codes, ICD diagnosis codes, an NPI, a place-of-service code or a claim number, and nothing here maps to a CMS-1500. A patient who needs to submit to an insurer or a personal-injury attorney should get a properly coded superbill out of your practice management system — sending this instead will usually get the claim rejected and the patient sent back to your front desk. Use it for cash patients, deductible-phase visits, prepaid plans, and anything else genuinely being paid out of pocket.
The notes field carries what has no field of its own: your license number and clinic tax ID, the range of dates the visits cover, the treatment plan the invoice belongs to, and the payment terms behind the due date. The currency preset is USD with no tax rate set, since professional services are commonly untaxed while goods sold over the counter — pillows, braces, supplements — may not be. Only one tax line is available and it applies to the whole subtotal after the discount, so check with an accountant before putting care and products on the same invoice.
Closing out a six-week care plan
A Boise clinic bills a cash patient at the end of an acute care plan. The eight adjustments are counted rather than listed one by one — the quantity column holds visits and the description carries the date range — while the intake examination and the two-view lumbar series are one-off charges that belong on their own lines at their own prices. The re-examination at week four is priced below the intake because it is shorter, and the cervical pillow taken home from reception is goods rather than care, which is why it sits at the bottom where the patient, and anyone reviewing the account, can see it separately.

Where visit-based billing goes wrong
Visits belong in the quantity column
Eight adjustments at one rate is a single line with 8 in the quantity column and the date range in the description — repeating the line eight times only makes the PDF longer.
Exams and imaging are not adjustments
Give the initial examination, any re-examination and each X-ray series their own lines at their own rates, because those are the charges a patient is most likely to question.
Say what the plan bought
For a prepaid care plan, name the number of visits it covers in the description and put the prepayment reduction in the discount field, then state in the notes what happens to visits left unused.
Keep goods apart from care
Pillows, braces and supplements are retail and are often taxed differently from treatment; list them last, and if the tax treatment differs, invoice them separately rather than forcing one rate across the whole subtotal.
Say plainly that it is not a claim
Add a line to the notes stating this is a patient statement with no CPT or ICD codes, and point the patient to the front desk for a coded superbill — otherwise it gets mailed to an insurer and bounces back.
