Itemizing private-pay dental treatment
Dental invoices are read tooth by tooth, so put the detail in the description: "Composite restoration — tooth 14, two surfaces" tells the patient far more than "filling" ever will. One line per procedure at your fee, quantity 1; where the same procedure repeats across teeth, either give each tooth its own line or use the quantity column and list the teeth in the description. Radiographs count naturally — four bitewings is quantity 4 at your per-image fee — and a hygiene visit, a fluoride application or scaling per quadrant each get a line of their own.
None of this is an insurance claim. There are no ADA CDT procedure code fields, no tooth-number or surface columns, no provider NPI and nothing that produces an ADA claim form, so claims and pre-treatment estimates have to come out of your practice management software. Use this for self-pay patients, for treatment a plan does not cover, and for balances left after benefits — with the treatment dates, your license number and the practice tax ID in the notes, since none of those has a field of its own either.
Lab work passed through at cost belongs on its own line, named as such — "Crown, monolithic zirconia, Meridian Dental Lab — passed through at cost" — so the patient can see which part of a crown fee is the laboratory's and which is the practice's. There is no deposit or part-payment field, so if the patient paid something on the day, either state the amount and date in the notes and invoice only the balance, or enter the full treatment and take the payment off as a fixed discount; say which you have done, or the patient will assume the other. The currency preset is USD with no tax rate set, since dental services are typically exempt while goods sold at the desk sometimes are not — check with an accountant if you sell products.
Two May appointments, itemized tooth by tooth
A Fort Collins practice invoices a self-pay patient across two appointments in May. Every procedure carries its tooth number in the description, because a patient reading the invoice a month later wants to know which tooth cost what. The four bitewings are counted in the quantity column at a per-image fee instead of being folded into an appointment charge, and the laboratory's invoice for the crown is passed straight through on a line of its own so the split between the lab's work and the practice's is visible. The hygiene appointment at the end of the month stands at its own fee, unrelated to the restorative work.

Write "tooth 14, two surfaces" rather than "filling" — there are no tooth or surface columns here, and the description is the only place that detail can live.
Lab work on its own line
Show the laboratory charge separately and say it is passed through at cost, naming the lab and the restoration; a crown fee with no visible lab split is the one patients push back on.
Radiographs counted, not bundled

Frequently asked questions
Can I submit this to a dental insurer?
No — insurers need ADA CDT codes, tooth numbers and surfaces in defined fields on a claim form, and this tool has none of them. Bill claims through your practice management software and use this for the self-pay portion or for patients with no coverage.
How do I show a lab fee?
Give it its own line, name the lab and the restoration, and say it is passed through at cost. Patients accept the number far more readily when the laboratory's share is visible rather than buried inside a single crown fee.
How do I invoice a balance after the patient has already paid something?
There is no deposit field, so either state the amount paid and its date in the notes and invoice only the remainder, or enter the full treatment and deduct the payment as a fixed discount. Whichever you choose, say so on the invoice so the patient can reconcile it.
Is dental treatment subject to sales tax?
In most US states dental services are not taxed, though products sold at the front desk sometimes are, and rules differ elsewhere. Leave the rate empty unless you know you have to collect it — one tax line here applies to the whole subtotal — and check with an accountant.
